 About the Dyslexia Assessment:

You have referred your child for a specific learning difficulty assessment for dyslexia.  The assessment aims to develop a detailed learning profile, identifying strengths and areas of need and to consider whether the profile is consistent with a diagnosis of dyslexia.  Finding may or may not result in a diagnosis. 

The assessment includes standardised testing, observation, background information, professional analysis and a written report. 

The assessment may be recorded to enable the assessor to review it during the report writing process.  It will be deleted once the report is approved.

The assessment will need to take place in a quiet, suitable space with minimal interruptions.

Equipment ordinarily used will need to be brought to the assessment.  For example, glasses, hearing aids, fidget aids and anything specific to their needs.

Please inform the assessor, in advance of the assessment, of any medical needs, adjustments or considerations that may need to be considered during the session.

Professional Standards and Limitations of the Assessment:

The assessor holds a valid APC with the British Dyslexia Association and diagnostic reports comply with SASC guidelines.

The report represents a professional opinion based on evidence available at the time of assessment.  The results reflect the performance on the day of testing and can be influenced by factors such as anxiety, fatigue or motivation.

An assessment does not guarantee a diagnosis of dyslexia.

The assessment is educational in nature and does not consitute medical, phychological or legal advice.

 Booking, Fees and Payment:

Assessments are booked via email. Once a date is secured a non-refundable deposit of £100 is required.  The remaining payment of £475 must be paid at least 48 hours before the assessment.

The fee includes; the assessment session and a written report with detailed recommendations.

The location of the assessment will be confirmed during the booking process.

 Appointment Cancellation and Rescheduling:

If you need to cancel or reschedule your appointment with more than 48 hours notice your deposit can be transferred to an alternative date.  If less notice is given the full fee may still be charged.  It is understood that unforseen circumstances such as emergencies or illness may arise. Every effort will be made to arrange an alternative appointment.

The deposit is non-refundable and will be retained in the event of cancellation.

Failure to attend without notice is chargeable in full.

If I need to cancel (due to illness or emergency) you will be offered a full refund or an alternative appointment.

 Consent to Obtain, Share and Discuss Information:

With email/written consent, relevant information may be obtained from or shared with schools and other professionals involved in your child’s education.  Information is shared only with explicit consent unless required by law or safeguarding duties.

Reports:

The assessment fee includes a PDF copy of the final report.  Reports are intended for use by parents/carers and educational professionals and must not be altered, partially reproduced or used out of context.  The assessor is not responsible for how third parties interpret or apply the report.

The written report will be provided within 21 days of the assessment.  It will include a clear summary of key findings, provide detailed analysis of assessment and practical recommendations for home and school.

 Data Protection and Confidentiality:

All personal data is handled in accordance with UK GDPR.

Assessment records and reports with be stored securely.  Any handwritten assessment material will be securely destroyed once the report has been finalised.

The final report and relevant digital records will be retained until the child reaches the age of 25, or in the case of adult assessments for 7 years.

Information will not be shared without your consent, unless required by law.

If you have any concerns or complaints about the assessment process or the service received, please email me and this will be dealt with promptly.
